  • New patient visit/consultation - $200

    Established patient visit:

    Low complexity $110 (<20 min)

    Medium complexity $130 (20-39 min)

    High complexity $150 (40-60 min)

Making the Most of Your Visit

Call to Schedule

Our front desk will collect your demographic information. Please notify us of any urgent need, high risk condition, or recent hospitalizations that may require priority scheduling.

Prepare for Your Visit

Collect records from your prior PCP if able.

Create a current allergy and medication list, consider bringing your medication bottles (preferred).

Know the approximate date and results of your last colonoscopy, pap smear, mammogram, dexa scan if applicable.

Gather a care team list - all of your physicians who are actively following your care.

Complete your intake paperwork (see your email provided during scheduling for forms)

Visit day

Please arrive 30 minutes prior to your appointment time for new patients, 15 minutes for established patients.

Items to bring:

-insurance card

-photo ID

-medication bottles (or list)

-care team list

-any healthcare records that you have

After Your Visit

Schedule your follow up visit and mark your calendar

Call us if you are notified of any need for prior authorization for medications, testing
